Just fill out the form with your email and enter your Metamask Polygon Wallet Address. Double-check to make sure that the wallet address before you hit "Claim".

Once the NFT is sent to your address, you will receive an update on the registered email.

The NFT may appear only in the "Hidden" section on Opensea and you may have to unhide it to be publicly visible.
